This form must be completed prior to riding at Windy Hill Sports. If the rider is 18 years or older, this form can be completed on practice day by the rider. If rider is less than 18 years of age, this form must be completed by the rider's parent or legal guardian. If the minor rider's parent or legal guardian is not present with the rider on practice day, this form along with our Medical Authorization form must be executed by the rider's parent(s) or legal guardian(s) prior to practice and brought with the minor rider on practice day.

Parent/legal guardian authorization of emergency medical services for their minor child. If the minor rider's parent(s) or legal guardian(s) are not present with the rider on practice day this form, along with our Minor Release and Waiver for Liability and Indemnity Agreement form, must be executed by the rider's parent(s) or legal guardian(s) prior to practice and brought with the minor rider on practice day.
